The puzzling and inconsistent ...

  1. Why is Occurrence (stage of life) not applied more widely: In Infantile, juvenile and presenile cataracts, Congenital cataract but not in Senile Cataracts or Juvenile Cataract. Why not use "old age" from life's stages?
  2. Classifier bug? Why are composites marked primitive?: Drug-induced cataract vs Cataract associated with radiation vs infrared radiation. Surely none are primitive but two of the three are said to be.
  3. Wrong Finding Site: Nucleus of lens concepts have finding site "structure of nucleus of lens" but Nucleur Cataract doesn't!
  4. One Left and right - laterality as an axis: Bilateral Cataracts is the only concept with laterality, unlike in ICD 10 which distinguishes laterality for all cataracts. Problem here is coordinating, not according to "len opacity" but according to "left lens opacity" etc. This demonstrates how coordination can explode but also why post-coordination is needed. Shouldn't bilateral be dropped for SNOMED CORE and post-coordination of laterality for particular cataracts be encouraged?
  5. Too much specialization? Corticosteroid induced cataract specializes Drug-induced cataract by naming a more particular drug. But why does this drug get special "pre-coordination" favor? Why not others?
  6. Child classed as peer : Localized traumatic opacity and Traumatic cataract are peers causing the latter to be classed as primitive.
  7. Cataract as Morphology!: Congenital lamellar cataract.
  8. Missing associations: Concussion Cataract has no concussion!
  9. Redundant or Misclassified: Effectively, the group Structure of cortex of len treats immature and cortical cataracts as synonyms.
  10. Vague specializations: Congenital cataract and lens anomalies - how would a problem list use this? It appears to exist just to be the parent of the primitive and otherwise unspecified Cortical and zonular cataract

  1. Diabetes is not a causative agent: Why association in some cases but cause in others? Example: Diabetic Cataract vs Drug-induced Cataract
  2. Progression/scales are not given attributes: this leads to inheriting primitives like Mature Cataract into Total traumatic cataract although some stages don't get primitives ala Partial resolved traumatic cataract
  3. Some drugs like 'Corticosteroids and derivatives' don't seem to be classified as such in the substance hierarchy which means diseases with these agents like Corticosteroid induced cataract must explicitly note that the substance is a 'Drug or medicament'.
